Bilingual Registered Nurse The Bilingual Registered Nurse is responsible for assessing and screening consumers for clinical eligibility for all ASAP programs. This includes State Home Care as well as community based MassHealth reimbursed services. The Registered Nurse conducts assessments and monitoring by a combination of telephone contact, video conferencing, and home visits. Schedule: This is a full-time, non-exempt (35 hour/week) position. We offer a hybrid remote schedule (1 day/week in the office) and the option of a flexible 4-day work week. Part-time and per-diem schedule is available as well. Additional duties may be assigned as required. Comply with state regulations in performing screening activities for MassHealth community based reimbursed services. Complete initial assessment and reassessments for consumers by a combination of telephone contact, video conferencing, and home visits. Assess potential consumers and determine eligibility for State Home Care Provide ongoing nursing oversight as required by each program. Provide back-up coverage for CSSM RN. Provide clinical support to the Client Services Managers and care managers. Provide health education and resources for care managers, consumers, their families and/or caregivers. Participate in interdisciplinary model of care management and discharge conferences. Provide crisis care management, prevention and intervention. Maintain accurate consumer information in case records and client services database in accordance with Executive Office of Elder Affairs, Mystic Valley Elder Services, and MassHealth standards. Compile and submit required reports. Participate in the orientation and training process for new staff. Participate in regular supervision meetings. Develop professionally through training opportunities. Participate in collaborative team model. Attend Agency, Client Services department, and team meetings as required. Qualifications: Fluent in Chinese/Mandarin Valid Massachusetts Registered Nurse license. Excellent verbal and written communications skills required in both English and Spanish A Bachelor's degree in nursing with three years clinical experience preferred, one of which should be in community-based nursing or an Associate Degree in Nursing and five years of community-based nursing experience or long-term care direct service experience. Working knowledge of community resources and reimbursement systems for health and social services. Strong interviewing and assessment skills. Strong interpersonal and organizational skills. Excellent written and verbal communication skills. Valid Massachusetts Driving License Private transportation. Computer skills: Proficient with MS Office Suite, Microsoft and entering data into a database. Working knowledge of Excel. Ability to use the Internet to conduct information searches. Knowledge of and experience working with older people and adults with disabilities preferred. Experience working in a community service setting preferred. Must be able to ascend and descend stairs and navigate in and out of consumer homes during inclement weather and be able to lift light objects up to 25 pounds.
